I did think Sarah would quickly deteriorate as her steroids was reduced. She wasn't able to eat. This is the kindest decision Josh could have made for Sarah, in time he will look back and realize this. Nivolumab & Ipilimumab is a very touch combo, even for the healthiest stage 4 patients.

It must be so hard though because now there's no hope. That's very hard to come to terms with.
 
The poor man looks so defeated. I'm pretty sure he was still hopeful / in denial because she kinda looked ok... although honestly if you watch Sarah's videos from a few months ago you could see how much she had changed (her voice, mannerisms)... honestly she sounded like a completely different person so yeah, while she didn't look too deteriorated in general, you could tell that the disease had progressed quite aggressively. This is so tragic. F cancer.
 
It's very sad that going off her treatment for a month in the failed attempt to get into the NIH clinical trial really accelerated the deterioration. I guess at that point that was the only treatment option and she had to at least give it a try but it was quite the ordeal with both of the trips to NIH.

Thursday Josh posted a response to a comment that he actually asked her that day if she was ready to let go and die and she gave him one of her are you f'ing kidding looks. He really did try to abide by her wishes to the very end. I hope she goes peacefully and sleeps with the angels soon.
 
Trying to think of a way to word this nicely, but really can't. Steroids mask the symptoms of dying.

I think the energy and coherence steroids provide also offer renewed hope, as artificial as the improvements really are. It is just so optimistic to see a love one rebound and make gains. Not for nothing, it leaves some last days full of happy times and memories before the inevitability and acceptance set in. We so often see such resistance and last futile efforts before they come to terms with the situation. I think hospice must be expert at this transition and shift.

Yes, I also think "defeated" is the right word for how Josh presented their decision.

She made a care plan with Josh and her sister last week that said she didn't want a feeding tube or IVs so hospice is their only choice. They are going to continue giving her steroids and aggressive amounts of pain meds.

Yes, I did find it strange. In one sense, he was honest by admitting it’s for money. But on the other hand, it sounded kind of awful, it’s like saying the channel is only for money! He didn’t say it was for anything more noble, like helping others by sharing her journey, or leaving memories. He went straight to the point that the channel is there for the money, so that Sarah can provide income for them.

Sarah actually worked up until a few weeks ago. I don't know how much they had to pay out of pocket for medical bills but they did have to pay for Josh and Braden to go on the useless trip to NIH.

They never had a huge amount of views until the last couple of months. They've had 3 million total views compared with Jenny's almost 14 million.

Josh has replied back to comments that he will be making another video explaining how they arrived at this decision. Someone actually asked if Sarah could be seen and he, rightfully so, said no but that she can be seen on her previous videos. I can't believe that someone actually asked that.
