The fact the cat keeps pissing tells me he’s either ill (potential UTI), he doesn’t have a litter box and she’s too pissed to let him out or he’s very stressed. Maybe she should stop being selfish and take him to the vet.

She's moaning about losing the 'old fit healthy me' but was she ever fit and healthy really? Maybe more healthy when she was about 14 and presumably wasn't smoking and drinking all the time. 25 years of poor lifestyle choices have caught up with her. Not being self righteous - I've got my own, but get real George. You need a massive lifestyle change for a few years to get yourself out of this one.
