I have never had 'augmentation ' and apologies of this offends anyone who has but, why does it look so painful? And stretched?
Ella's boobs and lips look so tight and stretched and painful.

Because usually when a woman has them done regardless of whether they are flat chested or not they have some natural breast tissue there so a more natural look can be achieved whereas men naturally have far less tissue there so the skin needs to stretch further
